Environmental Services (EVS) is the hospital department responsible for cleaning, disinfection, waste removal, and linen distribution across the facility — the recurring, every-shift work that keeps a building safe to occupy. From a design and construction standpoint EVS rarely gets a marquee suite; instead it lives in a distributed network of closets, soiled- and clean-utility rooms, holding rooms, and dock-adjacent staging that must be sized, plumbed, ventilated, and located correctly on every floor or the department fails quietly and continuously. This article covers the space program, room-by-room requirements, waste-stream segregation, soiled-utility and linen logistics, and the codes that govern them — the operational systems that move dirty material out and clean material in.
EVS, regulated medical waste (RMW) handling, and linen flow are often treated as afterthoughts in early programming because they consume no revenue and house no high-acuity equipment. That framing causes predictable, expensive failures. A nursing unit cannot be occupied without a janitor's closet, a soiled-utility room, and a clean-utility room per the Facility Guidelines Institute (FGI) Guidelines; an operating room cannot turn over without soiled-utility and EVS support adjacent to the surgical suite; and a building cannot pass survey if its waste storage, chute systems, or soiled-handling spaces violate infection-control and life-safety requirements.

Unlike Sterile Processing or the central plant, EVS is not one room — it is a kit of small rooms repeated on every occupied floor, plus a few centralized hubs. The recurring "support core" on a typical inpatient floor includes:

Programming should establish an EVS closet/soiled-core "rhythm" early — a repeatable support module that lands in the same relative position on each floor of the bed tower so that vertical chases, plumbing risers, and chute alignment stack cleanly. Misaligned support cores across floors are a frequent source of costly structural and MEP coordination problems discovered late in design.
There is no single code-mandated square footage for the EVS program as a whole; sizing is derived from FGI minimum-room requirements, unit counts, cart-fleet size, waste-generation rates, and the chosen logistics model (chute vs. cart vs. automated transport). The soiled-utility room is the most regulated EVS-adjacent space and the one most often built incorrectly. Its purpose is the temporary receipt, sorting, and holding of soiled and contaminated material — used instruments awaiting transport to SPD, soiled linen, bagged waste, and emptied bodily-fluid containers — pending removal from the unit.
